ACNM HomeBirth Position Statement

This is the American College of Nurse-Midwives current position on the safety and efficacy of homebirth. It includes a review of current research material which supports the Midwifery Model as the appropriate standard of care for healthy childbearing women.

This is the custom birthkit for River & Mountain Midwives. If you are planning to have your baby at home with us, order your kit by 34 weeks. Ordering instructions are available on the form.

Informed Disclosure and Consent for Homebirth

Download, print. Both parents must read and sign this document after your interview and bring it to your first visit. If you have questions or concerns about this document, bring them to your appointment before signing. This document includes our financial agreement. If you have made special arrangements with River & Mountain, these will be detailed at the time of your visit and written into your agreement.

Mana Consent

If you choose to participate in the Midwives Alliance of North America study of planned homebirth and the Midwifery Model of Care, please download, print and sign this document and return it to us at your next visit. This document must be received by MANA before you have your baby for your statistics to be included in the study.

Supply List

This is a list of items, in addition to your birth kit, which we ask you to assemble into one basket by 36 weeks. If you have any questions about items on the list, feel free to ask at your next visit.
